Evidence packs auditors accept without a second chase

Reviewers rarely ask for more volume. They ask for navigable proof that decisions happened when you said they did.

In Ledger Assurance Path we treat the evidence pack as a product with users: internal audit, external reviewers, and the future version of your own team who will reopen the file months later.

Minimum contents that earn trust

  • Scope note: entities, returns, and period covered
  • Extract inventory with refresh timestamps
  • Exception log with closed decisions, not just open tickets
  • Sign-off trail with names and roles, not shared mailboxes

What to leave out

Raw dumps that nobody can reconcile, screenshots without context, and email threads that bury the decision three replies deep. If a message contains the decision, lift it into the log; do not hope the auditor will hunt.

Timestamp discipline

Your regulatory filing audit app should stamp when an exception changed state. Manual backfills are fine occasionally — document them. Invisible edits are what create second chases.
